These terms govern your use of this website and the telehealth services provided by Elevra Optimal Health, LLC. Please read them before you enroll.

1. Agreement to these terms

This website is operated by Elevra Optimal Health, LLC, whose mailing address is 600 Furlong Ct, Wentzville, MO 63385. Care is delivered by telehealth; that address is not a clinic or walk-in location. In these terms, "Elevra", "we", "us", and "our" mean that company. "You" means the person using this website or receiving services from us.

By using this website, submitting a form, booking a consultation, or enrolling in a protocol, you agree to these terms. If you do not agree, please do not use the website or our services.

These terms do not describe how we handle information about you. That is covered in our Privacy Policy.

2. Not medical advice

The content on this website is for general informational purposes only and does not constitute medical advice, diagnosis, or treatment.

Reading this website, sending us a message, or booking a consultation does not create a provider-patient relationship. Nothing here is a substitute for advice from a licensed clinician who knows your history. Do not delay or disregard medical advice because of something you read on this site.

3. Telehealth services

Elevra is a telehealth clinic. All care is delivered remotely by providers licensed in Missouri. There is no in-person clinic visit.

Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911.

4. Programs, pricing and payment

Elevra is cash-pay only. We do not bill insurance and we do not submit claims to any insurer on your behalf.

Consultations are free. The initial consultation is 30 minutes and follow-up consultations are 15 minutes.

Lab work is billed separately and is not included in any monthly price.

Each protocol's monthly price covers the medication and supplies described on that protocol's page. Monthly protocols continue month to month.

The weight-loss protocols, Transform and Elevate, are dispensed one month at a time. You contact the clinic to re-order each month, and any continued dispensing remains subject to provider review. Pinnacle peptide therapy is dispensed by the vial and also requires you to contact us to re-order.

Founding-member pricing is limited to the first 75 new members. Regular pricing applies after that.

5. Appointments and cancellations

Consultations are by appointment, Monday through Friday, 9:00am to 6:00pm Central. If you need to move or cancel an appointment, please tell us as far in advance as you reasonably can so the time can be offered to another patient. Call (636) 697-4436 or email info@elevraoptimalhealth.com.

Programs are billed monthly and continue until you cancel. To cancel, call (636) 697-4436 or email info@elevraoptimalhealth.com before your next monthly billing date. We will confirm your cancellation in writing.

Medication that has already been dispensed or shipped cannot be returned, and fees paid for it are not refundable. If you believe a charge was made in error, contact us and we will review it with you.

6. Acceptable use of this website

You may use this website for lawful, personal purposes only. You agree not to:

Please do not send detailed medical information through website forms, email, or text message. Those channels are not intended for clinical information. Your clinical records are kept in our electronic medical record, and your provider will tell you where to send anything sensitive.

7. Intellectual property

The website and its content โ€” text, layout, photography, graphics, the Elevra name and logo, and the protocol names Ascend, Radiance, Balance, Complete, Transform, Elevate, and Pinnacle โ€” are owned by Elevra or its licensors and are protected by copyright, trademark, and other laws.

You may view and print pages for your own personal, non-commercial use. Any other use, including copying, republishing, distributing, or creating derivative works, requires our written permission.

8. Disclaimers

The website is provided on an "as is" and "as available" basis. To the fullest extent permitted by law, we disclaim all warranties, express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the website will be uninterrupted or error-free, or that its content is complete or current.

Medicine is individual. Care and response vary from person to person, and no particular result or outcome is promised.

Laboratories, pharmacies, and other third parties involved in your care are independent of Elevra, and we do not control their acts or omissions. Links to other websites are provided for convenience and are not an endorsement.

Nothing in these terms limits the professional or legal responsibilities of a licensed provider.

9. Limitation of liability

To the fullest extent permitted by law, Elevra and its members, employees, and contractors are not liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, lost data, or loss of goodwill, arising out of or relating to this website or these terms โ€” whether the claim is based in contract, tort, or any other theory, and even if we were advised that such damages were possible.

To the fullest extent permitted by law, our total liability for all claims relating to this website or these terms will not exceed the amount you paid to Elevra in the twelve months before the event giving rise to the claim.

Some jurisdictions do not allow certain exclusions or limitations, so parts of this section may not apply to you. This section does not apply to claims arising from professional medical services, which are governed by Missouri law.

10. Indemnification

You agree to indemnify and hold harmless Elevra and its members, employees, and contractors from claims, damages, losses, and reasonable legal fees arising out of your use of this website, your breach of these terms, or your violation of any law or of the rights of a third party.

11. Governing law

These terms are governed by the laws of the State of Missouri, without regard to its conflict-of-laws rules. Any dispute arising out of or relating to these terms or this website will be brought exclusively in the state or federal courts located in St. Charles County, Missouri, and you consent to the jurisdiction of those courts.

12. Changes to these terms

We may update these terms from time to time. The effective date at the top of this page shows when the current version took effect. Changes apply once they are posted, and continuing to use the website or our services after that means you accept the updated terms.
